Somatropin (recombinant human growth hormone, 191 amino acids) is a single-chain polypeptide structurally identical to the principal circulating isoform of pituitary-derived human growth hormone. Crystallographic studies of the hormone-receptor complex show that one growth hormone molecule engages two growth hormone receptor (GHR) subunits sequentially through two distinct binding interfaces, followed by a third receptor-receptor contact required for full activation; this engagement triggers GHR dimerization and downstream JAK2 phosphorylation. The GHR belongs to the class I cytokine-receptor superfamily and exists as both a full-length transmembrane form and a soluble extracellular growth-hormone-binding-protein form.
